Annual Forms

We are asking parents to verify/update their student information through the PowerSchool Parent Portal.  The Annual Update forms will be available from August 1st through September 15th. These forms are located in the Parent Portal under Forms in the left navigation window, and then under the General Forms tab. The forms included in the update are:

  • Health Information
  • Student Address
  • Student Contacts & Custody
  • Permissions/Agreements
  • Transportation Request Form

NOTE:  Even if you have a new student to the district, please review and submit the forms to assure that all data is accurate.

ADDRESS CHANGES WITHIN DISTRICT

If your address has changed within Anchor Bay School District, please be sure to update the Student Address form and provide us with two (2) proofs of residency and your photo I.D. for the new address.  Proofs of Residency can be uploaded through the form or they can be turned into your student’s school on or after August 18th.
